Timing is critical, so please pay careful attention to all requirements and due dates, and respond quickly to our requests for information and documents. If you can, continue making your regular mortgage payments until we notify you about possible mortgage assistance options available.

Download, print, and complete:

  • Mortgage Assistance Application with Expenses and Debt (PDF) - Explains the reasons for your hardship and verifies information about you, co-borrowers, and the property. It also includes a form to list your expenses and debt. 
  • IRS Form 4506-C (PDF) and Borrower Consent Form (PDF) - Authorizes the Internal Revenue Service (IRS) to provide a copy of a previously filed tax return. You may be asked to sign, date, and return these forms, which give us permission to verify the income you reported to the IRS in prior years.
    • Your information on these forms — such as your name and address — must exactly match the information on your tax form.
    • You must sign and date the forms.
  • Non-borrower Financial Contribution Form (PDF) - This form provides documentation for non-government-backed loan applications of who is living in the residence and how much the person is contributing financially to the home.

Tips for a smoother application process

  • Make sure documents are clear and legible
  • Don’t alter any pages (no strikethroughs, cross-outs, etc.)
  • Send all pages, even the blank ones
  • Be sure that all the information requested is included

You should also receive a package in the mail which may include some of the above referenced documents. You can use either version, but we recommend printing them now and returning them along with supporting documents.

Important notes

  • In order to ensure timely evaluation of possible mortgage assistance options, if you are required to submit documents, please make sure they are sent by the due date noted in the package you will receive by mail.
  • All borrowers must sign and date all documents where required.
  • Include your loan number on all correspondence.
  • You may be required to provide documentation that is dated within the last 30 days. If documentation is required, a letter will be sent to confirm next steps.
