Form I-94

A record created electronically when a foreign visitor enters the United States and is maintained in the United States Customs and Border Protection (CBP) system.

You may be asked to provide Form I-94 to help verify your employment authorization. You can obtain a record of your most recent I-94 admission at cbp.gov/I94.

The following may also be requested in these situations:

  • Permanent and non-permanent residents are required to have a valid U.S. Social Security number and U.S. credit history.
  • Permanent residents may have to provide an enlarged copy of a Green Card (officially known as a Permanent Resident Card).
  • Non-permanent residents may have to provide a copy of an eligible visa and a work authorization.

Tips for a smoother application process

  • Make sure documents are clear and legible.
  • Don't alter any pages (no strikethroughs, cross-outs, etc.).
  • Send all pages, even the blank ones.
